Abhira panicking and being worried for Ruhi makes full sense even if you leave out the sister factor. From a pregnant woman’s perspective, Abhira is feeling twice the emotions in comparison to others. She undergoing immense stress bc of this incident is normal, especially if something happens to the baby. This was the exact same reason why Goenkas and Singhanias kept Keerti’s accident a secret from Naira for as long as they could.

Just like how Ar jumped in fire to save Ru as he's a human not like thise vile people, same with Ab being worried about a fellow pregnant lady as high on emotions.
