Well as per the BTS the doctor is saying exactly that - ke Armaan should have guessed seeing Abhira's diet & symptoms

Ab doctor ko kaun samjhaaye ki Karwanchauth pe madam ne sargi pe ice cream & toast khaai aur chakkar khaakar girna madam ko purani aadat hai - last time toh she was deemed infertile with that incident, toh Armaan guess bhi karega kaise
