Apparently Adhya admitted herself into the foster care system in the US when she left Anuj's house and Megha and her husband were assigned as her foster parents. The scene when she was playing with a pen and said her parents are dead was probably when she was at a shelter home.

Megha possibly had a daughter of her own who looked like Adhya and is dead/missing which is why she is possessive about Adhya and doesn't want her to leave. She has cut her hair and renamed her to Priya and helicopters her all the time.

Megha had a daughter named Priya who was killed and raped 1 year ago and her face was same as adhya

so now megha is not letting aadhya go out ( The world is a bad place just a mother wants her daughter to be protected )

So she is delusional but well meaning, still better than saying "Choti Samajhdaar hai khud ko sambhal legi" for an 8 year old.
