I doubt that she will realise that Asfi saved her. It's more like she fears that he was in Karachi and somehow everyone in Karachi knows/talks about her badkirdar. This thought is just her own I believe.

Also I feel that Asfi will be the one to realize it first. He may go thru his old boxes and find her ID card within that or in a book from his medical school times. I feel he will get nostalgic about Daniyal and go back to Islamabad to visit their old home. All the memories of old times will come back to him. He will just start going thru his old stuff and suddenly come across Zubiya's ID card.

But one thing is he won't tell her that it was him. I feel he will give her a chance to tell her side of the story. He will most probably allow her to confide in him first. Like after they get close and she talks about her past life and tells him about this incidence, he will tell her it was him indirectly or he may give hints that zubiya will understand quickly.
