Sai and Virat express their feelings through their actions, not through their words. This is their biggest problem... because although actions normally speak louder than words, here they have the dreaded words of the vaada hanging over their relationship. There is no verbal communication of clarity. Their actions point in one direction, but their words say otherwise. This is why both of them are in a constant state of confusion as to what the other person feels. At this point, it is clear that neither one of them considers this marriage a deal, regardless of what they say to each other. Sai is going to choose to return to CN with Virat, because he is her home. That is why she was so shattered today, saying she feels as if she has no home. She did not consider that aai, Shiv Bua, Mohit, etc. are there for her...that they love her. To her, Virat is who matters most.
I agree that this is the perfect opportunity for them to end the vaada once and for-all. The confession will be stretched, but there is no reason that the deal marriage can't end in the hospital. In fact, ending the deal marriage is way more important to their relationship than any confession at this point. Once it ends, Sai will give herself permission to accept that she loves Virat and accept that Virat loves her. She is on the crux of this realization... in fact a part of her has already realized...it's just the label that is standing in the way.
However, all of this being said, I don't think that we can blame her if they return home with all of this unsolved and she once again brings up the zimidari. I agree that it will cause their relationship to go in circles, which will be frustrating, but it is not her fault at all... she is desperate to hear Virat tell her that this marriage is real, and until that happens, she won't be able to accept it as such.
