In real life, people regift things all the time unless there is some sentimental value attached to it. A gift reminds us of the person and/or the occasion when it was given to us. And If there is a sentimental attachment to it, the price of the gift doesn't matter. So for Sai, the pen with her name on it had much more sentimental value because it was given as something to start her new journey of her medical career with. But this ruby pendant was not perceived by her in that way. For Virat it was hus first gift after he realized his love for Sai, but Sai does not know that. Does NOT mean Sai is right. Once Virat told her that he was hurt she should've apologized. But unfortunately the conversation was set up in such a way that Sai was already upset with Virat so when he brought up his genuine hurt she didn't give it its due weightage just like Virat didn't give due weightage to her pain of being treated so poorly by his family. He feels sympathetic but he wants her to change rather than his family.
