Yes, feelings are there but more under the garb of her being Ishwari's doctor, her being a nice stand up girl, his guilt for how he brought her / her insults by Mami and may be ... may be her being the only person who can be far fetchedly called as his friend, who advises him, with whom he can laugh or may be open up and share things which he doesn't with anyone..

He does care about her, but currently he hasn't realized that this is not just care for his mother's doctor or to his employee or out of responsibility or humanity.. he hasn't realized that he cares so much because he likes her!! He doesn't realize it because he has never had time to understand this part of life, where he had chance or time to even think / spend so much time with a girl in a family / personal setting, having a different kind of personal equation.. the kind of which he has started to share with Sona.

It will take him time to understand the true meaning of his care and worry, may be the new entry track would lead him to that...As sona is about to realize her feelings, this new entry may make her feel jealous and may want to avoid dev thinking he also likes Kareena. Either this or Sona's health recovery & her lesser presence in his daily life may lead him to miss her pushing him towards the realization.
