To Shakti.. she looks like a stranger/a family friend who saved Shakti. After that Shakti might have started considering her as sister. 

I'm more confused about her relationship with Shiv. I hope it was related to Shiv's profession only. But that photo of Gauri with Shiv and earlier fbs are suspicious. 

I think khushi was just a stranger who saw shakti drowning in water near her house and saved came to the rescue..

for shiv i think she was his lover from the scooter flashback.. but other flashbacks seem like she was just a patient as she used to say me marna nai chahti "doctor"
