Yes, high risk high returns is the hustle culture, also true, no workplace is devoid of toxicity. 

Consultant salary is nowhere near a doctor's salary though. 

We're talking about shadow labour and people working over 60 hours for a job that pays them not enough to get a full night's sleep, it plays with one's mind. 

Anyways, not disagreeing with the basic principle/logic you present, it does work in theory.
