Actually it was the scene of the day. In fact after he questioned her relationship with JM, she completely blocked him. Even if there were emotions for him in her heart, they hardened at those demeaning questions. And then it was only a doctor who was treating the patient, there was no Sai who would have mellowed at seeing V in pain.
