Is this Sufi idea always so boy-sided? What about how it works from the girl's angle? Death seems a bit extreme, one always retains a sense of self even in love, otherwise it becomes an unhealthy obsession does it not?

Obsession, worshipping and madness are not true love. This is like Malini's feelings.

These things happen when love is unrequited, otherwise when the feelings are mutual the love grows harmoniously and when consummated in a true permanent relationship like marriage there is no need for the last three points on the list unless one of the couple changes their mind and leaves.
