DOB is not mandatory even though it is marked with a star, just leave it blank. For Siddharth, type Siddharth in the name and Ashok or Shukla in father's name. Select the district as Mumbai Suburban and constituency Versova. For Bhau, Vikas as first name and Pathak as father's name. Constituency is Vandre-West. His father's name is Jairam and residence in Danda, Khar so you can see the correct one.
