One of the main reasons that a serial can get out of hand is the station itself. For e.g. SunTV. What they should do is give each director - say about 600 episode to produce and finish and assign how many hour for air time. If the serial is popular, then they should be aired at peak time. But the limitations is 600 episodes or something to the effect.
Instead of which they give the directors 3 months to produce, after which they judge the ratings, if that serial is a success, they extend the air time. So by that time, the Director does not know what to do, so the story line changes, actors changes, etc. So that is how things get out of hand. The same happened in Muhutram. Initially the story line was good, although I hate it when the villian have the same line dialogue. A flaw here and there was acceptable but overall it started off well. Eventually the Directors in their own way managed to screw up things.
