The past timeline with the kid is messed up. What I think is maybe Ananya somehow escaped from her dad and ended up in orphanage and that nurse was treating her that time and then adopted her from there. Also I have a feeling sidharth/Shivani and Aditi are not related. Maybe they are the "good children" born to different mothers and circumstances/vanraaj forced them them in same place and maybe that orphanage fire was meant to end thek all but since they are the good ones they survived and it hurt vanraaj somehow . Like Voldemort was destroyed due to Lily's sacrifice. There has to be a reason why he was quiet for years until Vik found him. Was it a co incidence or deliberate thing? After Vik met Vanraaj both Aditi and Shivani started experiencing paranormal entities more than ever.
