I have purchased the filmfare magazine of May 11 2011 and uploading the image again along with two more pages, one page is about a letter on Madhuri and the other is some feedback about the issue that had her on cover.

Interesting, this is a magazine from 1993, Madhuri did exactly what her mother predicted. Though it took a while to get the right person. (ignore the stupid comments by the magazine itself, also it has a comment of sanjay dutt)
