Well even I had this thought in mind but then I joined a few scenes:

1. Bhade ke goons started working for Patralekha so that she wouldn't complain about their attempt to steal her necklace.

2. They repaid Patralekha's favour by attacking Sai. So here Patralekha and goons are even.

3. In today's precap, after the doc is killed, Patralekha is calm and composed, almost like she hired the men again.

4. Assuming Patralekha hired the goons. Why would any goon risk their lives if they are not getting anything?

5. Patralekha is not giving them money, then how is she keeping them happy and at her beck and call? ☺️

I think the attack was for Sai and doctor is collateral damage. Or it is Patralekha. I wish it is Patralekha. The darker her character becomes, there is a chance for her to be punished
