I think there won’t be anything major injury, Sai suffered life threatening injuries recently.

I think this creativity of Sai getting hurt by Ghum makers is just to make Devi and Aai go insane with their superstitious talk and stop Sai from leaving or just to make Sai ending up in same hospital as Sruthi that will lead to Sruthi and Sai face off and Virat prioritizing Sruthi even After knowing Sai in pain Eventually leading To divorce.
