Yes in some customs first names are changed like in Maharashtrian and Sindhis. Not sure about others.
So generally it would be Sai Kamal Joshi with her father's name (it's more because schools insist on a middle name for women which is generally father's if she's unmarried or husband's if she's married). Now if Sai changes her surname and takes on Chavan surname then by default her name would be Sai Virat Chavan during her admissions etc
In Christians, women have a second name that's their own name and they don't take father's or husband's first names as their middle name

No. In India official documents do not allow anyone to choose a middle name other than father or husband's first name. It can be left blank and is not optional. But there is no provision yet to include a woman's maiden surname and husband's surname jointly. Many women do it now out of awareness and choice but it is for all informal usage. Formal documents can have only one surname with or without middle name. It's not mandatory to change the surname, however.
