I don't think Mrunal and Malhar are the same person, and the person behind the blackmail could be this Mrunal itself... he wants Kalyani to hate Malhar and has the money to get Swara's cured, hence why Malhar is doing everything he's told by this person.

By the time this is over, something will happen that will be too much for even Kalyani to bear and it won't be too easy for Malhar to get back in her good graces, even with all the explanations about how he was forced to do it for his sister's life.
