I think, given by the preview for Tuesday, not even Malhar will know she's faking a more serious injury than then one she actually has, and this will make him restless and make it more difficult for him to pretend to hate her and keep his charade up in front of her. There may be a few romantic moments were he shows love and care for her like old times.
